You can choose Saline Breast Implants or Silicone Breast Implants for your breast augmentation. Silicone breast implants are popular because they are softer and feel more natural. There tends to be less visible rippling in patients with thinner breast tissue. Past media attention on silicone breast implants raised safety concerns; however, recent studies support their safety.  Saline breast implants have the advantage of being adjustable during implantation. This allows our plastic surgeons to correct any minor asymmetries during your breast augmentation surgery. Also, if a saline implant ruptures the saline is absorbed by the body and the deflated implant is obvious, indicating the need for replacement. Some disadvantages are that they are not as soft and sometimes cause visible rippling.

Plastic Surgeon Dr. Vennemeyer may recommend that your breast implants be placed under the muscle (subpectoral) or over the muscle (submammary) based on your anatomy. Women with thinner breast tissues often benefit from subpectoral breast augmentation, because the extra soft tissue coverage makes the breast implant less palpable and a more natural appearance results. Subpectoral breast augmentation may result in slightly more postoperative discomfort, but it will not limit normal activities after recovery. Subpectoral implants also have less chance of capsular contracture (scar tissue formation around the implant).

Submammary breast augmentation is typically only performed in patients with thicker pre-operative breast tissues. These thicker tissues provide more coverage of the implant making the muscle less important in gaining a good aesthetic result.

Breast augmentation is typically an outpatient operation. You will need to have someone take care of you for the first 24 hours. Your pain will be controlled with pain medication and you will need to rest and restrict your activity. You will need to wear a soft surgical bra for four to six weeks. You may resume light activities and work within a week. Refrain from any vigorous activities for approximately one month.

Bleeding can result from any surgery. A large collection of blood, called a hematoma, can form. This would need to be drained in the operating room.  Infection can occur. This requires treatment with antibiotics and sometimes requires removal of breast implants if the infection does not resolve with antibiotics. Breast implant leakage or rupture can occur at the time of surgery or anytime afterwards. Once this is recognized the implant would need to be removed and replaced. Capsular contracture is the formation of scar tissue around breast implants. This scarring may be soft and unnoticeable or it can become more firm and bothersome over time. If it becomes a problem, this may require surgery to remove the scar tissue. Placement of breast implants under the pectoral muscle decreases the chance of this happening.

How do I choose my breast implant size?

Based on your response and some specific measurements of your chest size and soft tissues, Dr. Vennemeyer will make a recommendation. You will then make the final decision after your questions and concerns have been addressed.

Should I choose Silicone Breast Implants or Saline Breast Implants?
This is a very personal decision that is best made after Dr. Vennemeyer has explained to you the advantages and disadvantages of each implant type. At your consultation, you will be able to touch and feel both types of implants. Dr. Vennemeyer may also make recommendations based on your specific circumstances, but the decision is ultimately yours.

Will future mammograms be affected by Breast Implants?
Mammographic screening in patients with breast augmentation requires special views called "Eklund views" to examine as much breast tissue as possible. When you have mammographic screening as directed by your primary care physician, tell them that you have breast implants and they will obtain the proper testing. The presence of implants can result in decreased visualization of breast tissue on mammograms which could alter cancer screening. However, diagnosis does not appear to be delayed and the stage of cancer at diagnosis does not seem to be altered by the presence of implants. Further, studies comparing women with implants to those without implants have demonstrated equivalent survival rates if they develop breast cancer. Whether or not you have breast augmentation, Dr. Vennemeyer recommends monthly self exams and annual breast examination by your primary care physician. Mammograms will be recommended by your primary care physician based on your age, personal and family history.
